New BI-RADS® Atlas edited by American College of Radiology (ACR) is available online with an electronic version since end of 2013. This atlas includes a lexicon for mammographic, ultrasound and MR breast images with recommendation for interpretation. In comparison with previous versions, the idea is to standardize mammographic, ultrasound and MR items. Some terms have been suppressed as some others have been added, for more precise description of images. Breast density categories have been changed on mammogram and ultrasound, and percentages of breast density have been suppressed. Microcalcification descriptors have been also modified with suppression of intermediate malignancy category; so, only two categories stayed in the lexicon, typically benign and suspicious. In the report, an important change concerns the distinction between assessment and management. Indeed, category assessment and management may be different, if it is concordant with clinical management. This review is the first of three articles summarizing changes in the 2013 BI-RADS® Atlas, and details changes in the mammography version.
