Secondary neoplasia is found with a higher frequency in patients with chronic lymphocytic leukemia (CLL), but the development of acute leukemia in these cases is extremely rare. We report on a 66-year-old man with CD5+ CLL, diagnosed in the second stage according to Rai. Eight months later, blasts were found in the peripheral blood and the bone marrow smears. The cytological exploration for the presence of two cellular populations, confirmed also by immunophenotyping, presented a combination of CLL with acute myeloid leukemia - M5 variant. Induction therapy on POACH scheme was conducted and a remission of the acute leukemia was achieved with persisting lymphocyte infiltration in the bone marrow. The patient died three months later with data for progression of the CLL and inflammatory complications.
