N2 - Background: A simple and reproducible surgical technique for gluteal reshaping and augmentation with fat grafting is needed. Objectives: The authors describe a novel approach to gluteal augmentation that combines simplified preoperative markings, extensive power-assisted liposuction of buttocks surroundings, and moderate volume of fat grafting to buttocks. Methods: This technique was performed in a total of 442 patients using a power-assisted liposuction and fat grafting to the gluteal region with simultaneous tunnelization and vibration. Results: The mean body mass index of the patients was 29 kg/m2. The mean liposuction volume was 2700 mL (range, 1400-5000 mL), and the mean fat injection volume was 500 mL per side (range, 280-900 mL). The mean operating time was 90 minutes (range, 60-120 minutes) when no liposuction of the abdomen was necessary. The complications included burning sensation in 22 of the 442 patients (5%), persistent edema in the lower back in 6 patients (1.35%), and mild infection in 2 patients (0.45%). Conclusions: Power-assisted gluteal augmentation with fat grafting is an efficient, safe, and reproducible procedure achieving a pleasant gluteal projection and contour.
